Service Agreement – Life Skills 4 Parents Membership Program Subscription
(Plan-Managed NDIS)

This agreement has been made between:

Life Skills 4 Kids Pty Ltd ABN 81 167 127 488 (we, us or our);

and

We agree to provide the following supports for a period of six months (6) months or on cancellation of membership fees, whichever occurs first.

The support(s) to be provided under this agreement is “The Life Skills 4 Parents Membership Program (Program)”.

1. Program Inclusions

The Program includes:

• (a) Monthly Zoom call;
• (b) Closed Facebook group for ongoing support, questions and answers; and
• (c) An online library of 10-minute self-paced mini-trainings in our Life Skills 4 Parents online portal.

2. Length of Program

The Program is offered for NDIS clients either month by month or for a Six (6) Month Membership. You can participate on an ongoing basis, but be aware that this is subject to NDIS approval.

(a) If you have a monthly membership:

• (i) Your membership will renew each month automatically unless you end this agreement under clause 5 below.
• (ii) At each monthly renewal we will take payment from your credit card or other specified payment method as your monthly Program fee; and
• (iii) You may terminate your monthly membership at any time with 1 months notice in writing to us.

(b) If you have a six month Membership:

• (i) We will contact you a month before your membership is due to expire and discuss renewal of the Program membership with you.
• (ii) Where you agree to renew your membership, we will automatically take payment from your credit card or other specified method of payment previously provided to us for the membership fee; and
• (iii) You may terminate your six month membership with 1 months’ notice at anytime during your membership period.

3. Claiming under your NDIS funding

If you intend to claim under NDIS for funding, it is your responsibility to sign, and forward to your plan manager or NDIS contact a copy of this NDIS agreement from Life Skills 4 Kids, as well as if you accept to extend your membership after each 6 monthly period of membership.

You acknowledge and agree that if you have registered for the Life Skills 4 Parents Membership, we do not guarantee that this will be covered by NDIS funding, and we recommend that you check under your child’s NDIS plan in this regard. If you are covered by NDIS funding, you have the sole responsibility for claiming back through NDIS and we do not assist in managing payments or claims in this regard.

4. Your participation in the Program

In participating in the Program, including any sessions or watching any of the online material or participating in the Facebook Group, you acknowledge and agree that:

• (a) while we do our best to provide support and training, the results may vary as each person is different and the results of the person we support will also depend on your commitment to the coaching and any advice or instructions that have been given to you by your coach for outside Sessions. We do not guarantee outcomes with respect to the results of our Sessions, including any improvement in general health and wellbeing;
• (b) We are not medical professionals and do not provide medical advice or mental health advice (Medical Advice). If you require any Medical Advice or advice related to other professional services we do not offer, we recommend that you seek a medical or other appropriate professional for advice.

Our membership terms and conditions are available here, and by signing you agree to abide by these terms.

5. Ending your Program participation

• (a) This agreement shall operate for the period for which you have subscribed to the Program (eg either month by month or for a 6 month period).
• (b) This agreement will be ongoing until you end your membership by giving us 1 month's notice in writing, your membership can be terminated by the ways set out in clause 2 of this service agreement.
• (c) We may end this agreement at any time, for any reason, with immediate effect by sending you notice in writing, noting that the membership will terminate immediately and the next monthly membership payment will be cancelled by us.
• (d) Termination by either party means that your right to participate in the Program immediately ceases.
• (e) We do not charge any penalties (or exit fees) for you to leave the Programs, however there shall be no reimbursement or credit if the Service is terminated due to your breach of this agreement.
• (f) We retain the right, at our discretion, to terminate any and all parts of the Services provided to you, without refunding to you any fees paid if we decide in our absolute discretion that you have failed to comply with any of the terms of this agreement.

(g) Agreement Signatures:

All parties understand and agree to the terms and conditions outlined in this service agreement.

I understand the hours agreed upon in my current agreement remain the same.
